Luteal phase support: definition and meaning

2 min read 路 Updated October 27, 2026

Definition

Luteal phase support is the use of extra progesterone, and sometimes estrogen or hCG, after ovulation or embryo transfer to help the uterine lining stay receptive for implantation and early pregnancy. It is standard in IVF, because the drugs used can weaken the corpus luteum, and is sometimes used in other fertility treatments. It usually continues until a pregnancy test or several weeks beyond.

Why it is needed in IVF

In a natural cycle, the corpus luteum makes progesterone after ovulation to prepare the endometrium. In IVF, the medicines used to control ovulation, and the egg retrieval itself, can leave the luteal phase short of progesterone. Adding it back improves pregnancy rates.

Forms of luteal support

FormNotes
Vaginal progesterone (gel, pessary, tablet)most common; can cause discharge or irritation
Progesterone injection (in oil)used in some clinics, especially for frozen transfers; injection-site soreness
Oral dydrogesteronea tablet option shown to work as well as vaginal progesterone in trials
hCG injectionsless used because of higher OHSS risk

Outside IVF

Some doctors offer progesterone after ovulation induction or for people with a history of early bleeding in pregnancy. The evidence is mixed for natural cycles. A so-called luteal phase defect is hard to diagnose, so routine progesterone is not recommended without a clear reason.

How it can feel

Progesterone can cause breast tenderness, bloating, tiredness and moodiness, very like PMS or early pregnancy. That makes the two-week wait especially confusing: symptoms don't tell you the result. Be gentle with yourself and lean on support.

Never stop prescribed progesterone without talking to your clinic. Call them for heavy bleeding, severe pain, or signs of an allergic reaction.

Frequently asked questions

Can progesterone support delay my period?

Yes. While you are taking it, a period may be delayed even if you are not pregnant, so test as your clinic advises.

Does luteal support prevent miscarriage?

It supports implantation in IVF. For people with recurrent bleeding in early pregnancy, progesterone may slightly improve outcomes; ask your doctor if it applies to you.
