Does birth control help PMDD?

3 min read 路 Updated October 26, 2026

The short answer

It can. Combined pills that stop ovulation remove the hormone swings that trigger PMDD. A pill with drospirenone and a low dose of estrogen is approved in the US for treating PMDD, and taking pills continuously, skipping the break, may help more. It doesn't work for everyone, and some people's mood gets worse on hormones. SSRIs are the other main first-line treatment.

Why birth control can help PMDD

PMDD is triggered by the brain's unusually strong response to the normal rise and fall of estrogen and progesterone after ovulation. Combined hormonal contraception (estrogen plus a progestin) stops ovulation, which flattens those swings. No ovulation, no luteal phase, and for many people, much less PMDD.

Which types have the most evidence

OptionWhat the evidence says
Combined pill with drospirenone 3 mg and ethinyl estradiol 20 mcg (24/4 regimen)Approved by the FDA for PMDD in people who also want contraception. A Cochrane review found it may help PMDD symptoms, though placebo effects were large too
Continuous combined pill (no break)Removing the hormone-free week may reduce symptoms that flare during it; recommended in ACOG's 2023 guideline as an option
Other combined pills, patch or ringMay help, less studied for PMDD specifically
Progestin-only methods (mini-pill, implant, hormonal IUD, shot)Don't reliably stop the cycle's mood swings; some people find mood gets worse
Give it time, and track it

It usually takes 2 to 3 cycles to judge whether a new pill helps. Rate your mood daily so you can compare honestly with how you felt before.

When birth control isn't the right fit

  • If you get migraine with aura, have had a blood clot, smoke and are over 35, or have certain other conditions, estrogen may not be safe. A doctor will check.
  • Some people find hormonal contraception lowers their mood. See birth control and mood.
  • If you want to get pregnant, it's not an option, and other treatments are used instead.

Other PMDD treatments

  • SSRIs: first-line, and they often work within days for PMDD, so some people take them only in the luteal phase.
  • CBT: helps with coping and has evidence for premenstrual symptoms.
  • Lifestyle: regular exercise, steady sleep and less alcohol and caffeine support other treatments.
  • GnRH medicines: for severe PMDD that hasn't responded, under specialist care.

Talk to a doctor before starting or changing hormonal contraception, and tell them if your mood gets worse on it. If you ever have thoughts of harming yourself, call your local emergency number now, or in the US call or text 988. In the UK, Samaritans are on 116 123.
